1 · News and Context

A massive new study by the ICMR has revealed a terrifying truth: standard antibiotics are failing to cure Indians. Worse, patients are not just getting these “superbugs” from popping too many pills at home; they are actually catching them inside the hospitals.
This highlights the severe crisis of Antimicrobial Resistance (AMR). Over 61% of hospital patients showed resistance to our strongest “last-resort” medicines. Because of poor hospital hygiene, these drug-resistant bacteria are spreading rapidly, leading to skyrocketing medical bills and much higher death rates.

2 · How the Superbug Crisis Unfolds

Step 1: The Last Resort Fails
Doctors keep strong antibiotics called “Carbapenems” as a final weapon. Shockingly, 61.1% of patients now have bacteria that easily survive these powerful drugs.
Step 2: Hospital Transmission
Over 85% of severe bloodstream infections were caught directly inside the hospital due to dirty wards, unhygienic surgical tools, and poor infection control.
Step 3: The Financial Drain
Because cheap medicines no longer work, patients are forced to buy incredibly expensive experimental drugs, pushing families into severe medical debt.
Step 4: The Deadly Outcome
With no medicines left to fight the bacteria, the death rate for these resistant infections is massive, sometimes crossing 50% for critical patients.

3 · Key Health & Policy Terms

Carbapenems
The Final Weapon
A class of highly powerful antibiotics used only when standard medicines fail. Losing these means we have almost nothing left to fight deadly bacteria.
Red Line Campaign
Stop Self-Medication
An awareness drive where strict prescription-only antibiotics have a visible red line on the packaging, warning patients not to buy them over the counter.
Schedule H1 Drugs
Strict Pharmacy Rules
A government rule making it illegal for chemists to sell certain strong antibiotics without a doctor’s prescription, requiring them to keep strict sales records.
NAP-AMR 2.0
The National Plan
India’s official plan (2025-2030) to fight superbugs using a “One Health” approach—tracking resistance across humans, animals, and city wastewater.

Prelims Quick Facts: The Data
Healthcare-Associated Infections (HAIs) Infections caught inside a medical facility. The ICMR study proved that solving the superbug crisis requires cleaning up hospitals, not just banning pills.
The Deadly Four The top bacteria causing havoc are E. coli, Klebsiella pneumoniae, Acinetobacter baumannii, and Pseudomonas aeruginosa.
The Financial Gap Treating a resistant bug costs significantly more. Even using cheap government pharmacies, medicine costs alone jump from ₹48,000 to over ₹66,000.

MCQ Practice Question
Q. With reference to Antimicrobial Resistance (AMR) and related government initiatives in India, consider the following statements:

  1. Carbapenems are first-line, mild antibiotics available over-the-counter for treating common viral fevers.
  2. Drugs listed under Schedule H1 of the Drugs and Cosmetics Rules require strict record-keeping and cannot be sold without a valid medical prescription.
  3. The ‘Red Line Campaign’ aims to discourage the irrational use and self-medication of antibiotics by marking their packaging.

Which of the statements given above is/are correct?
(a) 1 and 2 only    (b) 2 and 3 only    (c) 3 only    (d) 1, 2 and 3

Answer: (b) 2 and 3 only

  • Statement 1 — Incorrect: Carbapenems are not mild or first-line; they are powerful “last-resort” antibiotics used for severe bacterial infections, and they do not work on viruses.
  • Statement 2 — Correct: Schedule H1 was specifically created to legally curb the over-the-counter sale of critical, resistance-prone antibiotics.
  • Statement 3 — Correct: The Red Line Campaign visually warns consumers against buying and using strong antibiotics without consulting a doctor.
